Uterine perforation with subsequent migration of an intrauterine contraceptive device (IUCD) is a rare but serious complication. We report the case of a 29-year-old postpartum woman who presented with missing IUCD threads and intermittent right lower abdominal pain 1 month after insertion of a copper T380A IUCD. Ultrasound revealed an empty uterine cavity and a T-shaped echogenic structure in the right abdomen. Abdominal radiography and computed tomography (CT) confirmed extrauterine migration, with the device embedded in the omental fat at the L3-L4 level and no evidence of visceral injury. The patient underwent successful laparoscopic removal, and her recovery was uneventful. This case highlights the importance of multimodality imaging in timely diagnosis and management of IUCD migration.
