Synergistic analgesia: ultrasound-guided continuous suprascapular nerve block combined with oral gabapentin in managing frozen shoulder pain

Abstract

Abstract Introduction Adhesive capsulitis is a painful and disabling shoulder condition characterized by progressive restriction of both active and passive movements. Although suprascapular nerve block is widely used for pain control, optimal strategies to enhance and prolong its analgesic effect remain under investigation. This study aimed to evaluate the effectiveness of adding oral gabapentin to a continuous ultrasound-guided suprascapular nerve block in improving pain relief, range of motion, and patient satisfaction in individuals with frozen shoulder. Methodology In this prospective randomized clinical trial, 40 patients with unilateral adhesive capsulitis unresponsive to conservative management were allocated into two equal groups. The control group received continuous ultrasound-guided suprascapular nerve block alone, while the gabapentin group received the same intervention combined with oral gabapentin 300 mg once daily. Pain intensity was assessed using the Visual Analog Scale at rest and during movement, and shoulder range of motion was measured using a goniometer before, one hour after maneuver, and every week for six-week follow-up period. Results The VAS in Gabapentin Group during rest and active movements were statistically significantly lower than that in Control group from 1st week till 6th week. Also, the range of passive and active movement in Gabapentin Group was statistically significantly larger than that in Control from 1st week till 6th week. Patient satisfaction was notably higher in the gabapentin group. Conclusion The addition of oral gabapentin to continuous suprascapular nerve block provides enhanced and sustained pain relief, improves functional outcomes, and increases patient satisfaction. This combined approach may represent a more effective strategy for managing refractory frozen shoulder and supporting rehabilitation.
