Surgical Management of an Inferior Sinus Venosus Atrial Septal Defect with Pulmonary Artery Trunk Dilation and Right Pulmonary Artery Stenosis

Abstract

RÉSUMÉ Une communication interauriculaire est une ouverture dans le septum interauriculaire, cause d'un shunt de gauche à droite et d'une augmentation du volume de l'oreillette droite et du ventricule droit. Ce cas concerne un enfant de 12 ans avec une communication interauriculaire (CIA) de type sinus venosus, entraînant dyspnée et retard de croissance. Les examens paracliniques et l’exploration chirurgicale ont révélé une CIA large type sinus venosus, un shunt gauche-droite significatif, et des anomalies de l’artère pulmonaire. La prise en charge chirurgicale comprenait la fermeture de la CIA par un patch de péricarde autologue, avec vérification de la taille de l’artère pulmonaire droite sans élargissement nécessaire. L’intervention s’est déroulée sans complications, et les suites opératoires ont été favorables avec une récupération rapide et une amélioration clinique notable. Ce cas souligne l'importance d'une évaluation approfondie et d'une intervention ciblée pour éviter les complications à long terme dans les CIA complexes. ABSTRACT An atrial septal defect is an opening in the atrial septum, causing a left-to-right shunt and increased volume in the right atrium and right ventricle. This case involves a 12-year-old child with a sinus venosus type atrial septal defect (ASD), presenting with dyspnea and growth delay. Paraclinical assessments and surgical exploration revealed a large sinus venosus ASD, a significant left-to-right shunt, and abnormalities of the pulmonary artery. Surgical management included ASD closure with an autologous pericardial patch, along with confirmation of the right pulmonary artery size without the need for enlargement. The procedure was completed without complications, and postoperative recovery was favorable, with rapid recovery and marked clinical improvement. This case highlights the importance of thorough evaluation and targeted intervention to prevent long-term complications in complex ASDs.
