Surgical Management of a Congenital Curvature of the Penis : A Case Report

Abstract

ABSTRACT Introduction. Congenital curvature of the penis (CCP) is a rare condition associated with asymmetric development of the corpora cavernosa. Most studies refer to its management in adults. In children, it can be detected in the most severe cases, particularly in cases of chordae or short urethra. We describe a case of chordee without hypospadias in the context of a poly malformative syndrome managed in the urology department of Yaoundé Central Hospital. A patient aged 02 years 08 months, with no previous history, was brought to the clinic by his mother because of an abnormal appearance of the penis, a short penis and an abnormal urinary stream. The clinical examination led to the conclusion of a poly malformative syndrome associating a chordee without hypospadias and an umbilical hernia. Further investigations confirmed that the patient was male and that there were no other malformation. We surgically repaired the umbilical hernia and rectified the curvature, mobilising the urethra. The post-operative course was marked by persistence of the curvature at 15 degrees. CCP in children can often have significant psychological repercussions for the parents. A management algorithm would contribute to better management of CCP in these patients. RÉSUMÉ Introduction. La courbure congénitale du pénis (CCP) est une affection rare associée à un développement asymétrique des corps caverneux. La plupart des études portent sur la prise en charge des adultes. Chez les enfants, elle peut être détectée dans les cas les plus graves, en particulier en cas de chorde ou d'urètre court. Nous décrirons un cas de chorde sans hypospadias dans le cadre d'un syndrome poly malformatif pris en charge dans le service d'urologie de l'hôpital central de Yaoundé. Un patient âgé de 02 ans 08 mois, sans antécédents, est amené à la clinique par sa mère en raison d'un aspect anormal de la verge, d'une verge courte et d'un jet urinaire anormal. L'examen clinique a permis de conclure à un syndrome poly malformatif associant un chordee sans hypospadias et une hernie ombilicale. Les examens complémentaires ont confirmé que le patient était de sexe masculin et qu'il n'y avait pas d'autre malformation. Nous avons réparé chirurgicalement la hernie ombilicale et rectifié la courbure, en mobilisant l'urètre. Les suites opératoires ont été marquées par la persistance de la courbure à 15 degrés. La CCP chez l'enfant peut souvent avoir des répercussions psychologiques importantes pour les parents. Un algorithme de prise en charge contribuerait à une meilleure gestion de la PCC chez ces patients.
