Abstract Background Testosterone plays an important role in the modulation of erectile function. Recent studies support there are strong effects of testosterone on the anatomical and physiological substrate of penile erection. Studies have demonstrated that erectile dysfunction is up to three times more prevalent in individuals with Metabolic syndrome. Phosphodiesterase-5 inhibitors have been shown to essentially improve erectile function in different randomized, placebo-controlled clinical trials. The study aims to evaluate the effect of once-daily tadalafil 5 mg on testosterone level in patients with erectile dysfunction accompanied by metabolic syndrome. Methods Prospective nonrandomized Trial was conducted on 60 Egyptian patients who have erectile dysfunction and metabolic syndrome. All the patients received tadalafil 5 mg once daily for 6 months. Erectile function was assessed using the five-item version of the International Index of Erectile Function (IIEF) questionnaire. All participants have three or more criteria of metabolic syndrome. Serum total testosterone was evaluated before starting tadalafil and 3 and 6 months after daily tadalafil intake, blood samples were taken between 08.00 and 10.00 in the fasting state. Results A total of 60 men with metabolic syndrome were evaluated for ED in this study. At the end of 3 and 6 months, mean testosterone values showed an improvement from baseline values 4.25(±1.22} to 4.71(±1.15) and 5.11(±1.14) at 3 and 6 month, respectively. There was significant difference in serum total testosterone values (p < <0.001). Conclusion Once-daily tadalafil 5 mg significantly increased total testosterone level in patients with erectile dysfunction and metabolic syndrome.
