Resolvins as Biomarkers in Chronic Obstructive Pulmonary Disease: Assessing Disease Severity

Abstract

<bold>Introduction :</bold> Recent research indicates that unresolved inflammation is a significant contributor to Chronic Obstructive Pulmonary Disease (COPD) pathogenesis. Resolvins have garnered attention as potential therapeutic agents. Our study aimed to evaluate plasma resolvin levels in COPD and their correlation with disease severity. <bold>Methods :</bold> We conducted a case-control study at our pulmonology department between March and September 2022, involving 40 male patients diagnosed with COPD in stable condition. This group was matched with control subjects. Patients with COPD-associated respiratory diseases and those receiving immunosuppressive treatment were excluded. Participants underwent interviews, physical examinations, and disease severity assessments using the Global Initiative for Chronic Obstructive Lung Disease 2022 (GOLD). Blood samples were collected for analysis of Resolvins (D1, D5, and 7,17 hydroxy-Docosapentaenoic acid) and C-reactive protein (CRP) levels, alongside forced airway measurements. <bold>Results :</bold> The average age was 64 years ± 8 among patients and 59 years ±7 among controls. According to the ABCD assessment tool, patients were distributed as follows: GOLD A (5%), GOLD B (55%), GOLD C (20%), and GOLD D (20%). No significant association was observed between COPD risk and SPMs levels. Among SPMs, only Resolvin D1 showed a correlation with peripheral oxygen saturation (r=-0.390, p=0.013). Remarkably, a significant negative correlation was found between 7,17-hydroxy-DPA and Forced Expiratory Volume in the first second (r=-0.434, p=0.005). <bold>Conclusion:</bold> Our study identifies resolvins as promising biomarkers and links to COPD severity, enriching our understanding of the disease's mechanisms.
