P1290 Prevalence and Risk Factors for Extraintestinal Manifestations in Moroccan Inflammatory Bowel Disease Patients

Abstract

Abstract Background Extraintestinal manifestations (EIMs) significantly affect prognosis and quality of life in patients with inflammatory bowel disease (IBD). There are no large-scale data from Morocco describing their prevalence and associated risk factors. Methods We conducted a cross-sectional study between 2016 and 2023 at a tertiary care center in eastern Morocco, including 715 consecutive patients diagnosed with IBD: Crohn’s disease (CD, n = 415), ulcerative colitis (UC, n = 231), and IBD-unclassified (IBDU, n = 63). EIMs were defined as the presence of ≥1 of the following: • Musculoskeletal (spondyloarthritis, arthritis, psoriatic arthritis) • Ocular (uveitis, episcleritis) • Skin manifestations (erythema nodosum, pyoderma gangrenosum) • Hepatobiliary (primary sclerosing cholangitis, autoimmune hepatitis) Multivariable logistic regression was used to identify independent associations with age, sex, diagnosis, and disease duration. Results Among 715 IBD patients, the overall prevalence of EIMs was 14.3%: CD 16.1%, UC 12.6%, IBDU 9.5%. In multivariable analysis, risk of EIMs was higher in Crohn’s disease vs UC (OR 1.99, 95% CI 1.18–3.36, p = 0.010) and increased with age (OR 1.026 per year, 95% CI 1.010–1.042, p = 0.001); female sex (OR 1.13, 95% CI 0.73–1.77, p = 0.58) and disease duration (OR 0.97, 95% CI 0.93–1.01, p = 0.086) were not significant. Distribution of EIMs: musculoskeletal 6.7%, ocular 2.0%, skin 1.3%, hepatobiliary 4.9% Conclusion In this large Moroccan IBD cohort, approximately 1 in 7 patients presented with an EIM, with higher risk in Crohn’s disease and in older individuals. Disease duration showed a slight, non-significant inverse trend with EIM occurrence. These findings highlight the importance of systematic screening for EIMs, particularly in Crohn’s disease.
