Outcomes of Combined Calcaneal Drilling and Plantar Fasciotomy in the Treatment of Resistant Plantar Fasciitis

Abstract

Background: Plantar fasciitis is the most prevalent cause of heel pain, accounting for approximately 11–15% of all foot complaints. Resistant plantar fasciitis is a debilitating condition unresponsive to conservative management. This study evaluates the effectiveness of combined partial plantar fasciotomy and calcaneal drilling. Methods: A prospective study included twenty patients (ages 33–56) with chronic plantar fasciitis refractory to conservative treatment for at least 6 months underwent the combined procedure. VAS and AOFAS scores were recorded pre- and postoperatively. Outcomes were classified as excellent, good, or poor. Results: Postoperatively, 20% of patients reported excellent, 45% good, and 35% poor outcomes. Mean VAS improved from 8.1 to 3.1, and AOFAS from 54.2 to 83.1. Conclusion: Combined partial plantar fasciotomy and calcaneal drilling provide effective symptom relief and functional recovery in most patients with resistant plantar fasciitis. This technique may offer improved healing by addressing both the mechanical and biological components of the condition
