Periportal tuberculous lymphadenitis with obstructive jaundice is rare and may mimic malignancy. We report a case of obstructive jaundice caused by a periportal tuberculous mass confirmed by fine-needle aspiration and GeneXpert. Treatment was complicated by anti-tuberculous drug-induced liver injury. Tuberculosis should be considered in the differential diagnosis in endemic settings.
