Abstract Acinic cell carcinoma (ACC) is salivary neoplasm of low grade of malignancy. It occurs most frequently in the parotid gland. Recurrences and metastases have been frequently reported in cases of ACC of the parotid gland. Brain, cavernous sinus, lungs, sternum, spine, liver, orbit, and skin remain the most frequent metastatic locations. Cases of metastatic ACC of the parotid gland to the pleura are extremely rare. We report the case of a 44-year-old male with a known history of ACC of the right parotid gland, who presented 5 years later, with dyspnea which revealed a pleural metastasis.
