Integrated management of cardio-renal-metabolic syndrome: Egyptian multi-society expert consensus and clinical practice algorithm

Abstract

Abstract Background Cardio-Renal-Metabolic (CRM) syndrome represents a major public health challenge in Egypt, contributing significantly to local morbidity, mortality, and healthcare expenditure. Existing international management guidelines lack localized, structured frameworks suitable for deployment within the Egyptian healthcare landscape. Methods A modified executive consensus process with binary voting was conducted among ten leading Egyptian experts representing the Cardiovascular Research, Education & Prevention Foundation (CVREP), the Egyptian Society of Diabetes and Lipidology (ESDL), and the Egyptian Society of Nephrology and Transplantation (ESNT). A predefined agreement threshold was set at 85%. The panel evaluated 10 core CRM consensus domains covering disease taxonomy, screening, and stage-specific management pathways. Results Unanimous agreement (100% consensus, 10/10 voters) was achieved across all 10 clinical domains during the initial voting round; consequently, additional rounds were not required. The panel translated these consensus domains into 10 clinical recommendation statements and synthesized them into a centralized, actionable healthcare algorithm tailored for both primary care and specialty settings. Key directives mandate systemic biological screening—utilizing estimated Glomerular Filtration Rate (eGFR) and Urinary Albumin-to-Creatinine Ratio (UACR)—alongside Social Determinants of Health (SDOH) assessments. The consensus formalizes the prioritization of cardioprotective and renoprotective guideline-directed medical therapies (GDMT), establishing dedicated implementation rules for Sodium-Glucose Cotransporter-2 (SGLT2) inhibitors and Glucagon-Like Peptide-1 (GLP-1) receptor agonists. Conclusions This Egyptian Multi-Society Consensus provides a practical, evidence-aligned framework designed to dismantle clinical siloing in the management of Cardio-Renal-Metabolic syndrome. By nationwide distribution of a unified algorithm to both GPs and referral specialists, paired with comprehensive educational workshops, public awareness campaigns, and standardized laboratory testing protocols, this initiative establishes a seamless, shared care model across Egyptian healthcare sectors. Systematically auditing provider adoption and diagnostic completion rates will ensure that this framework delivers measurable improvements in early detection, timely GDMT optimization, and long-term patient outcomes.
