Tibial avulsion fractures of the posterior cruciate ligament (PCL) are rare, and chronic cases progressing to non-union or pseudoarthrosis are exceptionally uncommon. We present the case of a 40-year-old woman with posterior knee instability caused by an 8-month-old pseudoarthrosis of the tibial insertion of the PCL. Surgical fixation was performed with a favorable clinical outcome. This case highlights the diagnostic challenges and imaging features of chronic PCL avulsion injuries and underscores the importance of surgical management in achieving joint stability.
