BACKGROUND: When women dropped from health facility delivery after ANC-booking, they missed proper care that could enhance their own health and that of their newborn. The aim of this study is to systematically review the available literature to determine the pooled prevalence and associated factors of dropouts from health facility childbirth among ANC-booked women in Ethiopia. METHODS: Electronic database including the PubMed, Scopus, SinceDirect, Web of Science and Google Scholar were systematically searched. Studies published up until March 12, 2025 were considered. We adhered to the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) checklist. All analysis was conducted using Stata statistical software and data were pooled using random-effects model. The risk of publication bias was checked by visualizing Funnel plots and Egger's regression statistical tests. Subgroup analysis was also conducted. A P-value of less than 0.05 was considered statically significant. RESULTS: = 99.2%, p < 0.001). Factors that were associated with dropout from health facility childbirth after NC-booking were maternal educational status, availability of health facility within 30 min, and exposure to media. CONCLUSION: In Ethiopia, nearly half of women who attended antenatal care did not proceed to a health facility for childbirth. It is crucial to prioritize evidence-based project planning and collaborating closely with media and employing local communication strategies.
