Abstract Background Nano-fat is one of the richest sources of adipose-derived stem cells and other progenitor cells. This work aimed to assess Nanofat injection in chronic ulcers and evaluating its healing process. Methods This prospective randomized controlled study was carried out on 20 patients aged above 18 years old and less than 60 years old with no co morbidities, both sexes, with chronic ulcers with no healing for at least 3 months with conservative management, small sized ulcers maximum diameter of 5 cm, upper and lower limb traumatic ulcers. Patients were divided in to two equal groups: Group A: Scheduled for Nanofat injection. Group B: No plan for surgical intervention Results Collagen formation, re-epithelization, neovascularization, Vascularity, pigmentation, and height were significantly improved in group A than group B. Time frame for complete healing was significantly delayed in group B than group A. The median time for complete healing in Group A was 3 weeks, while in Group B was 16.5 weeks. Satisfaction was significantly increased in group A than group B. skin biopsies showed that epidermal thickness, the area % of elastic fibres and the number of proliferating cell nuclei of the group A were increased compared to the untreated ulcers of group B. Conclusions Nano fat grafting may be a promising treatment option for patients with traumatic ulcers, as it can improve wound healing outcomes and patient satisfaction.
