Evaluate the efficiency of the IOTA and O-RADS-US classifications in predicting the malignancy of ovarian masses by verifying anatomo-radiological concordance Between April 2022 and December 2022, a retrospective longitudinal study included 28 patients with adnexal masses who underwent surgery in the Gynecology and Obstetrics Department of the Mongi Slim Hospital in Tunisia. First, each lesion was assigned to an O-RADS US and an IOTA category according to the criteria. Then we checked the histological type of the mass. 27 patients were included in our study. The mean age of the patients was 49, and 11 of them were menopausal. The adnexal mass was discovered on a routine pelvic ultrasound in 40.7% of the patients. The clinical examination revealed abdominal pain in 25.9%, (n = 7) pelvic mass in 18.5% (n = 5) and was normal in 55.6% (n = 15) of the cases. Vaginal pain on palpation was found in 2 cases. The analyse of the pelvic ultrasound images according to the IOTA criteria was benign in 63% of the images, malign in 25.9% and unclassified in 11.1%. According to the O-RAD US classification 13 masses (48.1%) were classified O-RADS US 2, 4 (14.8%) O-RADS US 3, 5 (18.5%) O-RADS US 4 and 5 (18.5%) O-RADS US 5. 13 images were classified benign and 5 malign according to both classifications. Among the 5 images classified O-RADS US 4, 2 were malign and 3 were unclassified according to the IOTA criteria. The anatomopathological examination revealed 74.1% (n = 20) benign masses among them 13 were classified O-RADS 2, 4 O-RADS 3 and 3 O-RADS 4, and 17 benign according to the IOTA criteria, 1 malign and 1 unclassified. 25.9 (n = 7) were malign and appears as an O-RADS 4 in 2 cases, O-RADS 5 in 5 cases and according to the IOTA criteria, 6 were malign and 1 unclassified. The IOTA and O-RADS US classifications are efficient in predicting the risk of malignancy of adnexal masses.
