Background The aim of this study was to assess the prevalence of depression and anxiety among Type 2 DM (T2DM) patients with comorbidity in Northwest Ethiopia. Methods A cross-sectional study was carried out with 409 patients with T2DM in Northwest Ethiopia. To determine the factors associated to depression and anxiety, a multivariable logistic regression model was applied. Variables with a p-value of less than 0.05 at a 95% CI were deemed statistically significant. Results The prevalence of depression and anxiety was 50.9% and 36.2%, respectively. Being female (AOR = 2.92, 95% CI: 1.75, 4.88), unmarried (AOR = 2.19, 95% CI: 1.30, 3.71), duration of illness greater than 10 years (AOR = 2.18, 95% CI: 1.16, 4.11), family history of mental illness (AOR = 2.20 95% CI: 1.30, 3.72), substance use (AOR =2.14, 95% CI: 1.18, 3.87) and poor social support (AOR = 2.96, 95% CI: 1.39, 6.31) were significantly associated with depression. Female gender (AOR = 1.91 95% CI: 1.15, 3.17), family history of mental (AOR = 1.85, 95% CI: 1.15, 2.99) substance use (AOR = 1.89, 95% CI: 1.11, 3.21) and poor social support (AOR = 2.68, 95% CI: 1.22, 5.86) were significantly associated with anxiety. Conclusion This study indicated that the level of depression and anxiety was notably elevated. Female gender, a family history of mental illness, substance use, and poor social support had a significant association with both depression and anxiety. Marital status and illness duration were significantly associated with depression.
