BRIDGING THE POST-DISCHARGE GAP: THE IMPACT OF A NURSE-LED INTERVENTION-BASED HELICOBACTER PYLORI SCREENING ON POST-OPERATIVE OUTCOMES AND ANXIETY IN SURGICAL ONCOLOGY PATIENTS

Abstract

Background: Gastrointestinal malignancies present complex challenges in surgical oncology. Helicobacter pylori (H. pylori) infection is a critical risk factor that often impairs post-operative recovery, potentially leading to peptic ulceration and delayed tissue healing after discharge. Nursing professionals are instrumental in facilitating the transition from clinical settings to home care, ensuring the continuity of recovery. Aim: This study aimed to evaluate the impact of a nurse-led intervention-based H. pylori screening on postoperative outcomes and anxiety among surgical oncology patients. Methods: A quasi-experimental research design was employed, utilizing a convenient sample of all 100 adult patients (50 intervention; 50 control) scheduled for gastrointestinal oncological procedures within six months. The study was conducted at the Sohag Cancer Institute and its affiliated surgical departments. The intervention group underwent pre-operative H. pylori screening (urea breath or stool antigen tests) followed by a structured nursing program encompassing pharmacological guidance and nutritional counseling. The control group received routine surgical nursing care. Post-operative monitoring was conducted at outpatient clinics in 30-day intervals. Data were collected using the Post-Discharge Surgical Recovery Scale (SRS), Visual Analogue Scale (VAS) for GI symptoms, Medication Adherence Rating Scale (MARS), the Patient Readiness for Hospital Discharge Scale (RHDS), and Tool IIV: State-Trait Anxiety Inventory (STAI). Results: Findings demonstrated that the intervention group experienced significantly fewer post-operative gastric complications (p < 0.05) and a marked reduction in 30- and 90-day readmission rates and anxiety levels compared to the control group. Furthermore, targeted nursing education and outpatient monitoring significantly enhanced patient compliance with eradication therapy. Conclusion: Implementing systematic H. pylori screening in surgical oncology patients effectively "bridges the gap" between acute hospital care and long-term recovery and anxiety levels reduction. Such interventions optimize patient safety, mitigate readmission risks, and enhance the overall quality of life for cancer survivors.
