Iatrogenic pneumothorax is most frequently descripted in the literature, but the association with pneumo-mediastinum and pneumoperitoneum is rare We report a case of bilateral suffocating pneumothorax associated with pneumo-mediastinum and pneumoperitoneum following a per oral endoscopic myotomy for Chicago-type I Achalasia managed by urgent exsufflation and a drainage.
