RÉSUMÉ Introduction : Le VIH entraîne des complications oculaires sévères lors d’immunodépression avancée. Ce travail expose les aspects épidémiologiques et cliniques des atteintes oculaires dans un groupe de patients camerounais VIH positifs sous TARV. Méthodologie : Une étude transversale de 4 mois a inclus des patients VIH+ sous TARV (>5 ans) dans deux hôpitaux camerounais. Les données sociodémographiques, cliniques et biologiques (CD4, charge virale, PCR CMV) ont été analysées par tests chi² et t de Student (p < 0,05). Résultats : Sur 400 patients, 99 (24,8 %) présentaient des lésions oculaires (âge moyen : 47,4 ± 11,3 ans ; sex-ratio H/F : 1/1,9). Les lésions prédominantes étaient des cicatrices de choriorétinite toxoplasmique (15,2 %) et des nodules cotonneux (12,1 %). Un taux de CD4 < 200/mm³ (41,9 %), une charge virale élevée et une PCR CMV positive étaient significativement associés aux complications oculaires (p < 0,05). Conclusion : Un quart des patients VIH+ sous TARV présentent des atteintes oculaires, principalement liées à l’immunodépression. Un suivi ophtalmologique systématique est crucial pour préserver leur qualité de vie. ABSTRACT Introduction: Human immunodeficiency virus (HIV) can lead to severe ocular manifestations in cases of advanced immunosuppression. This study reports the epidemiological and clinical aspects of ocular damage in a group of HIV-positive Cameroonian patients on antiretroviral therapy (ART). Methods: A 4-month cross-sectional study was conducted in two Cameroonian hospitals, including HIV-positive patients on antiretroviral therapy (ART) aged >5 years. Sociodemographic, clinical, and biological data (CD4 count, viral load, CMV PCR) were analyzed using chi-square and Student’s t-tests (p < 0.05). Results: Among 400 patients, 99 (24.8%) had ocular lesions (mean age: 47.4 ± 11.3 years; M/F ratio: 1/1.9). Predominant lesions included toxoplasmic chorioretinitis scars (15.2%) and cotton wool spots (12.1%). Low CD4 counts (<200/mm³; 41.9%), high viral load, and positive CMV PCR were significantly associated with ocular complications (p < 0.05). Conclusion: Nearly a quarter of HIV-positive patients on ART exhibit ocular lesions, primarily linked to immunosuppression. Systematic ophthalmological monitoring is essential to improve their quality of life.
