Aim: to assess the success rate of inferior alveolar nerve block (IANB) with 4% articaine (1/100000 epinephrine) containing 1% magnesium sulphate (MgSO4) in cases suffering from acute irreversible pulpitis (AIP) related to mandibular molars.Methodology: 50 cases suffering from (AIP) were carefully selected, then divided into two groups, the control group who were injected with 1.8 ml articaine (1/100000 epinephrine) for IANB while the second group (test group) who were injected with 1.8 ml articaine (1/100000 epinephrine – 1% MgSO4). Lip numbness was considered a sign of anesthesia work within 15 minutes. During the access cavity preparation and initial files placement, pain score was recorded according to Parker Visual Analog Scale. Results: Data analysis was done by Chi-Square and t-test. Success rate for the test group (MgSO4 group) was 85% and 55% for the control one. Conclusion: mixing 4% articaine (1/100000 epinephrine) with 1% MgSO4 had a positive value for IANB success in cases with (AIP) related to the mandibular molars.
