Background The eighth most deadly cancer in men in 2020 was prostate cancer (PCa), which killed 375 304 men. One of the many applications of MRI is to help with biopsy selection and to make it easier to target lesions during the procedure directly. Staging tumor extent and monitoring treatment response are two more areas where it proves useful. The Prostate Imaging Reporting and Data System (PI-RADS) scoring system, in conjunction with multi-parametric MRI (mp-MRI), allowed for a more precise diagnosis of PCa. Aim To determine the efficacy of mp-MRI using the PI-RADS in the detection of PCa. Patients and methods Our study enrolled 52 cases suspected of having PCa, who were referred from the urology department at Urology and Nephrology Center, Faculty of Medicine, Mansoura University. Results Comparing benign and malignant lesions in this investigation, we found that PI-RADS and mp-MRI (T2, apparent diffusion coefficient, dynamic contrast, and diffusion weighted image) showed significantly different results ( P = 0.001* and (0.001*, 0.001*, 0,002*, and 0.001*), respectively). PI-RADS sensitivity was 93.55%, specificity was 66.67%, and accuracy 82.69%. Conclusions Applying the PI-RADS using mp-MRI added a higher significant value in PCa detection and evaluation, by a novel, noninvasive imaging tool with high sensitivity and accuracy.
