The current evidence for thrombolysis’ efficacy beyond the usual treatment window is uncertain. Here, a comprehensive evaluation of the quality of evidence was performed for tenecteplase, a fibrin‐specific thrombolytic agent with a long half‐life, in AIS patients who presented after 4.5 hours from the onset of stroke.
