A Rare Case Report of Primary Laryngeal Diffuse Large B-Cell Lymphoma

Abstract

A 53-year-old man with a significant smoking history presented with progressive hoarseness, breathing difficulty, dysphagia, and persistent foreign body sensation in the throat for seven months. A clinical assessment revealed a mass at the base of the tongue. Cervical CT Tomography revealed a supraglottic lesion predominantly involving the epiglottis, extending to the base of the tongue, along with associated cervical lymphadenopathy. Histopathological and immunohistochemical evaluations of the biopsy samples confirmed the diagnosis of diffuse large B-cell lymphoma (DLBCL), with positivity for CD20 and CD45. Disease staging using 18-FDG PET and thoracoabdominal imaging established stage IIE according to the Ann Arbor criteria. The patient underwent four cycles of R-CHOP chemotherapy, followed by targeted radiotherapy (40 Gy in 20 fractions). A complete response was achieved after two cycles, with sustained remission at the one-year mark. The treatment was well tolerated. This report highlights the importance of a multimodal approach for managing primary laryngeal diffuse large B-cell lymphoma (DLBCL), a notably rare clinical entity.
